  • Request for College Transportation Services Form

  • Requests for the use of College Transportation Services must be submitted using this form, a minimum of twenty-one (21) calendar days in advance of the departure date. All requests for the use of College transportation services must be approved by an authorized College official identified below. The College Plant Operations Office will schedule appropriate College transportation services.

    Authorized Budget Approvers:

    • Athletics
    • AVP/Liberal Arts
    • AVP/Continuing Education and Workforce Development
    • AVP/Student Engagement and Wellness
    • Exec. Associate to the President & Board of Trustees

    User Responsibilities: 

    • Prior to departure, all passengers are required to complete an Excursion Waiver and Release.
    • All trip leaders are responsible for ensuring that all passengers participating on the excursion (whether they use College transportation services or not) have completed the Excursion Waiver and Release before departure.
    • All Excursion Waivers and Release forms must be documented by the trip leader on the Excursion Roster.
    • Trip leaders must provide a copy of the completed excursion roster to College Plant Operations immediately prior to departure and must take a copy with them on the trip.
    • Prior to departure, trip leaders must provide all participants with their cell phone number, and remain accessible at that number for the duration of the excursion.
    • Trip leaders are expected to return all vehicles in a clean condition by removing trash, bottles, etc. from seats and aisles prior to departing the vehicle.
    • Driver and passengers must wear seat belts, if available.
    • Use of tobacco products and vaping are prohibited in all vehicles.
    • Transportation or possession of alcohol, weapons, or illegal drugs is prohibited.
    • Students and employees are representing the College at all times during trip, travel, activities, and events. All College policies and procedures apply.
    • All Athletic rules and regulations apply for athletic travel.
    • Students are not allowed to drive College vehicles.
    • Only employees with a current Commercial Driver’s License (CDL) with passenger endorsement will be allowed to drive College vehicles.
    • Upon completion of the trip all excursion waivers should be submitted to the authorized budget approver.

    Cancellations/Changes:

    Must be made to the College Plant Operations Office at 301-846-2494 by the trip leader or requestor as soon as possible to ensure transportation providers are notified. Trip leaders should be persistent in getting cancellation messages to the College Plant Operations Office or if necessary, to the Special Assistant to the Director of Athletics (athletic trips), or the Assistant Director, Office of Student Leadership (student trips). The Plant Operations Office Manager will provide confirmation of the cancellation/change with the trip leader. Any cancellation/change made by the College will be communicated by the Plant Operations Office Manager to the trip leader and/or the requestor immediately.
